OBJECTIVES/GOALS: The purpose of this mixed methods project was to gain a comprehensive understanding and generate data on factors, including stress and inflammatory biomarkers, that may negatively impact glycemic levels in children aged 8-12 years with type 1 diabetes (T1D) from underrepresented backgrounds. METHODS/STUDY POPULATION: This study employed a two-phase sequential QUAN -> qual mixed methods design. Children and their parents were recruited from a pediatric endocrinology clinic in the southeastern United States. In phase 1 (n=34), we used quantitative methods to measure perceived stress, diabetes distress, cortisol, inflammation (IL-1b, IL-2; IL-6; IL-8; TNF-a; CRP), and glycemic level (HbA1c). Both children and their parent/guardian completed surveys, and children provided salivary and blood samples to measure cortisol and inflammatory markers. Phase 2 qualitative interviews in a subset (n=20) of children and parent/guardians from phase 1 are ongoing; preliminary findings will be included in the presentation. RESULTS/ANTICIPATED RESULTS: Mean age of children was 10.47 (sd=1.44), 67.6% were male, and 41.2% were black. HbA1c ranged from 6.8%-15% and only 2 (5.8%) children met ADA recommendations for HbA1c of 7% or less. HbA1c was associated with child reports of perceived stress (r = .403, p < .05), but not parent reports of child perceived stress (r = -.011, p > .05). Parent reports of perceived stress and diabetes distress in children were not significantly associated with child self-report of perceived stress (r = .11, p > .05) or diabetes distress (r = .018, p > .05). Exploratory models with PROCESS suggest that cortisol slope and IL-8 moderate the relationship between child’s perceived stress and glycemic control. DISCUSSION/SIGNIFICANCE: Stressors are emerging that are unique to this population and may help highlight disparities in care. While the study is ongoing, findings may help health professionals identify and mitigate stressors in children with T1D to help maintain optimal glycemic levels.

Aims
The coronavirus disease 2019 (COVID-19) pandemic and ensuing restrictions have negatively affected the mental health and well-being of the general population, and there is increasing evidence suggesting that lockdowns have led to a disruption of health services. In March 2020, South Africa introduced a lockdown in response to the COVID-19 pandemic, entailing the suspension of all non-essential activities and a complete ban of tobacco and alcohol sales. We studied the effect of the lockdown on mental health care utilisation rates in private-sector care in South Africa.
MethodsWe conducted an interrupted time-series analysis using insurance claims from 1 January 2017 to 1 June 2020 of beneficiaries 18 years or older from a large private sector medical insurance scheme. We calculated weekly outpatient consultation and hospital admission rates for organic mental disorders, substance use disorders, serious mental disorders, depression, anxiety, other mental disorders, any mental disorder and alcohol withdrawal syndrome. We calculated adjusted odds ratios (OR) for the effect of the lockdown on weekly outpatient consultation and hospital admission rates and the weekly change in rates during the lockdown until 1 June 2020.
Results710 367 persons were followed up for a median of 153 weeks. Hospital admission rates (OR 0.38; 95% confidence interval (CI) 0.33–0.44) and outpatient consultation rates (OR 0.74; 95% CI 0.63–0.87) for any mental disorder decreased substantially after the introduction of the lockdown and did not recover to pre-lockdown levels by 1 June 2020. Health care utilisation rates for alcohol withdrawal syndrome doubled after the introduction of the lockdown, but the statistical uncertainty around the estimates was large (OR 2.24; 95% CI 0.69–7.24).
ConclusionsMental health care utilisation rates for inpatient and outpatient services decreased substantially after the introduction of the lockdown. Hospital admissions and outpatient consultations for alcohol withdrawal syndrome increased after the introduction of the lockdown, but statistical uncertainty precludes strong conclusions about a potential unintended effect of the alcohol sales ban. Governments should integrate strategies for ensuring access and continuity of essential mental health services during lockdowns in pandemic preparedness planning.

Summary
While the health systems in Australia, New Zealand and other developed countries are regarded as some of the finest in the world, there is an ever-present need to ensure flexibility regarding cultural competence and responsiveness and cultural inclusivity across a range of practice settings. If current rates of immigration to Australia continue to grow, it is estimated that by 2050 approximately one-third of Australia’s population will be overseas-born (Cully and Pejozki, 2012).This chapter examines the mental health needs of people from refugee and immigrant backgrounds, with emphasis given to asylum seekers. Mental health issues that may affect these populations are explored, as is engagement between people of refugee and asylum seeker backgrounds and mainstream mental health services. This chapter seeks to deepen and broaden readers’ understanding of the effects of trauma among people of refugee background, and links this to strategies that might be used by mainstream mental health practitioners and services in response.

Objective:
Investigate an outbreak of coronavirus disease 2019 (COVID-19) among operating room staff utilizing contact tracing, mass testing for severe acute respiratory coronavirus virus 2 (SARS-CoV-2), and environmental sampling.
Design:Outbreak investigation.
Setting:University-affiliated tertiary-care referral center.
Patients:Operating room staff with positive SARS-CoV-2 molecular testing.
Methods:Epidemiologic and environmental investigations were conducted including contact tracing, environmental surveys, and sampling and review of the operating room schedule for staff-to-staff, staff-to-patient, and patient-to-staff SARS-CoV-2 transmission.
Results:In total, 24 healthcare personnel (HCP) tested positive for SARS-CoV-2, including nurses (29%), surgical technologists (25%), and surgical residents (16%). Moreover, 19 HCP (79%) reported having used a communal area, most commonly break rooms (75%). Overall, 20 HCP (83%) reported symptomatic disease. In total, 72 environmental samples were collected from communal areas for SARS-CoV-2 genomic testing; none was positive. Furthermore, 236 surgical cases were reviewed for transmission: 213 (90%) had negative preoperative SARS-CoV-2 testing, 21 (9%) had a positive test on or before the date of surgery, and 2 (<1%) did not have a preoperative test performed. In addition, 40 patients underwent postoperative testing (mean, 13 days to postoperative testing), and 2 returned positive results. Neither of these 2 cases was linked to our outbreak.
Conclusions:Complacency in infection control practices among staff during peak community transmission of SARS-CoV-2 is believed to have driven staff-to-staff transmission. Prompt identification of the outbreak led to rapid interventions, ultimately allowing for uninterrupted surgical service.

Objective:
Healthcare personnel (HCP) were recruited to provide serum samples, which were tested for antibodies against Ebola or Lassa virus to evaluate for asymptomatic seroconversion.
Setting:From 2014 to 2016, 4 patients with Ebola virus disease (EVD) and 1 patient with Lassa fever (LF) were treated in the Serious Communicable Diseases Unit (SCDU) at Emory University Hospital. Strict infection control and clinical biosafety practices were implemented to prevent nosocomial transmission of EVD or LF to HCP.
Participants:All personnel who entered the SCDU who were required to measure their temperatures and complete a symptom questionnaire twice daily were eligible.
Results:No employee developed symptomatic EVD or LF. EVD and LF antibody studies were performed on sera samples from 42 HCP. The 6 participants who had received investigational vaccination with a chimpanzee adenovirus type 3 vectored Ebola glycoprotein vaccine had high antibody titers to Ebola glycoprotein, but none had a response to Ebola nucleoprotein or VP40, or a response to LF antigens.
Conclusions:Patients infected with filoviruses and arenaviruses can be managed successfully without causing occupation-related symptomatic or asymptomatic infections. Meticulous attention to infection control and clinical biosafety practices by highly motivated, trained staff is critical to the safe care of patients with an infection from a special pathogen.

We present an exploration of the integrated stellar populations of early-type galaxies (ETGs) from the ATLAS3D survey. We use two approaches: firstly the application of line-indices interpreted through single stellar population (SSP) models, which provide a single value of age, metallicity and abundance ratio. And secondly, by fitting a linear combination of SSP spectra to our data, smoothly weighted in the free parameters of age and metallicity, thereby inferring a star-formation history of these galaxies. Despite the significant differences in these approaches, we obtain generally consistent results, such that galaxies that are more massive appear older with enhanced abundance ratios using line indices, and have shorter star-formation histories weighted to early times. We highlight two limitations of the index-SSP approach. Firstly the SSP-equivalent ages belie the fact that ETGs are overwhelmingly composed of ancient stars. Secondly, the young stellar contributions implied in our star formation histories are required to obtain realistic UV-optical colours. We remark that, even fitting solar-abundance models, we can recover a star-formation duration that correlates with the measured alpha-enhancement, in agreement with other recent work.

Summary
Introduction
The development of automated external defibrillators (AEDs) enabled the potential life-saving benefit of rapid defibrillation to be extended into locations outside traditional boundaries. Defibrillation with these devices could now be provided by minimally medically trained persons such as firefighters and police officers. The survival benefit from AED deployment by such users led to the presumption that even more rapid defibrillation might be provided by placement of AEDs in public settings where large numbers of persons are present, and where defibrillation might be accomplished by even less-trained persons, and possibly even by persons not trained at all in AED use. And thus emerged the initiative known as public access defibrillation, or PAD. In this chapter, experience with PAD is described in several different settings, fortunately with acquisition and reporting of data to permit analysis of outcomes. Experience to date is surely encouraging, yet questions remain. Pell and colleagues have raised such questions pertaining to cost-effectiveness and have recommended expansion of first-responder defibrillation such as by police or firefighters and bystander cardiopulmonary resuscitation as more defensible options to PAD.
In the PAD Trial reported by Peberdy in this chapter the low number of events is disheartening in light of the magnitude of the commitment in terms of numbers of persons trained and devices deployed, and the multiple locations in which AEDs were deployed. This observation may reflect yet another reality: the incidence of ventricular fibrillation (VF) as the presenting rhythm in out-of-hospital arrest settings is declining at an impressive rate, as reported now by several cardiac arrest investigators.

In January 1989 highway workers encountered human skeletal remains in a gravel quarry in south-central Idaho near the town of Buhl. Excavation revealed the remains of a young Paleoindian woman, 17–21 years of age at the time of death, with craniofacial attributes similar to other North American Indian and East Asian populations. She was buried in windblown and colluvial sediments immediately overlying Bonneville flood gravel. Grave goods include a large stemmed biface, an eyed needle, and a bone implement of unknown function. Isotopic analysis suggests a diet of meat and fish, including anadromous fish. Radiographs show numerous periods of dietary stress throughout the woman's childhood. AMS (accelerator mass spectrometry) dating indicates an age of 10,675±95 B.P., and geomorphological studies verify this single radiocarbon date suggesting it is the burial's minimum age. Following Idaho State law, the skeleton was claimed by the Shoshone-Bannock tribes of Idaho and reburied.
